Transfer Treatment Forms
If you have a current treatment plan at another facility but would like to begin treatment with MOUD treatment, we can take over right where you are in the process. There is no need to start all over. Please fill out the transfer forms and our team will begin the steps of transferring your treatment to our clinic.
